Purchase Order Form – *New Revision
As you may be aware there is a new Purchase Order Form (Revision 2_0). Please delete your old PO templates and use
the new form. The Department Webpage links have already been updated and point to the new form.
The reason for the change is there is now a field in the body called “CHEMICAL LOCATION”. This field is used for
ordering chemicals. Please include the end storage location for the ordered chemicals and Chemistry Stores Receiving
will create/update the EH&S Chemical Database and print a label for that purchase; removing that task from your group.
Purchase Order Form Revision 2
There were several additional changes that included formatting changes, data entry changes, and adding a couple
header fields.
