INTO Oregon State University offers employees a comprehensive benefits package. Our company has developed this comprehensive set of fringe benefits to supplement our employee’s regular wages.  Our fringe benefits represent a hidden value of additional income to our employees.

INTO OSU BENEFITS PLAN

INTO OSU’s health insurance plan provides employees and their dependents access to Medical, Dental, Vision, Life/ADD, and Long-Term Disability Insurance benefits. In addition, employees have the option of purchasing additional Life, Accidental Death, and Short-Term Disability benefits coverage. Regular full-time employees are eligible to participate in the health insurance plan the first of the month following their date of hire, or after their 90 day introductory period. Dependent eligibility criteria:

  • Your lawful spouse
  • Your unmarried dependent child under the age of 26
  • Your child with a mental or physical disability, as defined by the medical, dental plans who is over the age limit
  • Your domestic partner with a “certificate of registered domestic partnership”

Enrollment for benefits (medical, dental, vision, STD, LTD and life/ADD) must be completed within 30 days of your eligibility date in order for you to receive coverage. Your coverage is effective the first of the month following your hire date or the date you become eligible for benefits and your completion of enrollment paperwork. Employees will receive a benefit package when they are hired. After initial enrollment, changes may be made only when a qualified family status change occurs (i.e. birth, death, marriage, divorce, employment status change). Failure to enroll in benefits during this initial enrollment period will result in a six-month wait before enrollment in the plan becomes effective.

Employees with other group health (including medical/dental-vision) coverage may elect to “opt out” of the INTO OSU group health insurance plans and receive a “cash-back” benefits stipend. The opt-out amount is $233.00 per month, which is added to the participating employee’s pay and considered taxable income. See the HR Manager for the appropriate request form.

Full details of the health insurance plan are described in the Plan Overview and Summary Plan Description (SPD) available from Human Resources. An SPD and information on cost of coverage will be provided in advance of enrollment to eligible employees.

The company reserves the right to modify its benefits at any time. We will keep you informed of any changes.

Medical Coverage

For the following year, INTO OSU is offering a PPO plan through United Health Plan.  Preferred Provider organization (PPO) plans offer medical services and benefits at two levels of coverage – from preferred providers and from non-preferred providers. When you are in a PPO, you may use any doctors you wish, whether they are preferred providers or not. If you use doctors who are preferred, you pay less when you receive care. If you use providers who are not preferred, you pay more.

Vision Coverage

Vision coverage for employees and their families is provided by VSP. VSP’s Vision Care Program provides access to a nationwide network of private practice optometrists and ophthalmologists, as well as conveniently located retail providers. Through their provider network, you will receive a complete eye examination, as well as eyeglass lenses and frames, or contacts. You will receive most services at no additional cost, above any applicable co-pays.

Dental Coverage

INTO OSU offers a standard dental plan offered by Guardian. Guardian is one of the largest, strongest and most highly rated financial services companies in the world. Guardian offers worldwide coverage. The Guardian plan offers coverage with any licensed dental provider. The plan offers the choice to pay less for your care if you use preferred dentists who have contracted with Guardian. Employees have the ability to utilize a dentist who is not on the preferred list; however you will pay more for the service when you use a non-preferred dentist. This coverage also includes orthodontia for children up to age 19.

Life Insurance & Accidental Death & Dismemberment Coverage

INTO OSU provides Life insurance and Accidental Death & Dismemberment insurance from Lincoln. INTO OSU will provide Life Insurance equal to 1x your basic annual earnings to a maximum of $230,000. This policy also provides Accidental Death and Dismemberment (AD&D) insurance which would pay an additional benefit, up to the amount of your Life benefit, if you suffer a covered loss due to an accident. Benefits are reduced to 67% at age 65 and to 50% at age 70. Coverage is discontinued at termination of employment or retirement.

INTO OSU offers Voluntary Optional Life Insurance and Accidental Death benefit coverage in an amount between $10,000 and $200,000, not to exceed 3x your basic annual earnings. Employees have the option of purchasing spouse and dependent children benefits also. These benefits are contributory, meaning that you are responsible for paying for all of the cost through payroll deduction.

Disability Insurance

INTO OSU offers a Short Term Disability plan (STD) to all eligible employees at their expense. Short Term Disability plans offer income protection during times when an employee is unable to work due to an injury or illness. If an employee is injured or sick (this includes time off after giving birth), the employee will be able to access the short-term benefits of 60% of his or her weekly salary, up to $2,000/week, for up to 13 weeks.

INTO OSU offers Long-Term Disability (LTD) to all eligible employees. Long-Term disability will begin after an elimination period of 90 days of absences due to a covered accident or sickness at 60% of monthly earnings up to a maximum of $8,000 per month. This benefit is contributory, meaning that you are responsible for paying for all of the cost through payroll deduction.

Flexible Spending Accounts (FSA’s)

Employees may establish Flexible Spending Accounts (FSAs) to use pre-tax income to pay for eligible healthcare, dependent-care, qualified mass transit, and/or parking expenses. These are IRS-sanctioned accounts that allow you to authorize monthly deposits into your account from your pre-tax earnings. As you incur eligible expenses, you request tax-free withdrawals from your account to reimburse yourself. You may enroll for the accounts within 30 days of your initial date of hire, or during open enrolment which is August 1st of each calendar year.

 RETIREMENT SAVINGS PLAN

401(k) Savings Plan

INTO OSU has established a 401(k) savings plan. The 401(k) savings plan allows employees to elect how much salary they want to contribute and direct the investment of their plan account. To be eligible to join the 401(k) savings plan, employees must be 18 years of age or older. Employees may join the plan the first of the month following six months of employment. INTO OSU, in its sole discretion, may choose to contribute an additional matching amount up to 6% of each employee’s contribution once the employee has completed six months of service with INTO OSU.

ACCRUAL AND USE OF LEAVE TIME

Paid Leave Summary

Regular full-time employees are eligible for paid time off (PTO). PTO is intended to provide employees paid time off for opportunities for rest, relaxation, personal pursuits, sickness, disability, and vacation.

Regular full-time employees are eligible to earn and use paid time off as described in this section. The amount of paid leave that employees receive each year increases with the length of their employment as shown in the following schedule:

Eligible employees begin accruing leave from their date of hire and may request leave after completing 90 days of service

The length of eligible service is calculated on the basis of a “benefit year.”  This is the 12-month period that begins when the employee starts to earn vacation time. Employees can request use of paid time off after the successful completion of 90 days of service from the first day of employment. Upon termination of employment, employees will be paid for unused PTO leave that has been earned through the last day of work, including days accrued from previous calendar years. However, if INTO OSU, in its sole discretion, terminates employment for cause, forfeiture of unused PTO leave may result.

Parental or Family Medical Leave

The Federal Family & Medical Leave Act (FMLA) and the State of Oregon Family Leave Act (OFLA) provide qualified employees up to 12 weeks of unpaid leave per year for the birth or adoption of a child, to care for a seriously ill family member for personal serious health conditions, and to care for a child with a non-serious health condition requiring home care (OFLA only). Within the 12-week time frame, employees use accrued paid leave before taking unpaid leave. INTO OSU is subject to OFLA. INTO OSU is not subject to FMLA.

Regular full-time employees who have completed one year of service for INTO OSU will retain health care benefits, paid for by INTO OSU. If the employee does not return to work after taking the allotted parental leave, the employee will be liable to pay back these benefits to INTO OSU.

Oregon Domestic Violence Leave

INTO OSU provides a domestic violence leave of absence without pay to eligible employees who wish to take time off from work duties if he/she is a victim of domestic violence, sexual assault or stalking or is a parent or guardian of a minor child or dependent who is a victim.

Military Leave

A military leave of absence will be granted to employees who are absent from work because of service in the U.S. Uniformed Services in accordance with the Uniformed Services Employment and Reemployment Rights Act (USERRA).

Personal Leave

INTO OSU provides leaves of absence to eligible employees who wish to take time off from work duties to fulfill personal obligations. Regular full-time employees are eligible to request personal leave. Employees are required to use any available paid time off during this leave. Personal leave may be granted for a period of up to 30 calendar days every one year. Requests for personal leave will be evaluated based on a number of factors, including anticipated work load requirements and staffing considerations during the proposed period of absence. Benefit accruals, such as vacation or holiday benefits, will be suspended during the leave and will resume upon return to active employment. When a personal leave ends, every reasonable effort will be made to return the employee to the same position, if it is available, or to a similar available position for which the employee is qualified. However, INTO OSU cannot guarantee reinstatement in all cases.

Bereavement Leave

Employees who wish to take time off due to the death of an immediate family member should notify their supervisor immediately. Up to three days of paid bereavement leave will be provided to eligible regular full-time employees.

HOLIDAYS

Paid Holidays

INTO OSU will grant paid holiday time off to all regular full-time employees from their date of hire. Holiday pay will be calculated based on the employee’s straight-time pay rate (as of the date of the holiday) times the number of hours the employee would otherwise have worked on that day.

INTO OSU will grant holiday time off to all eligible employees on the holidays listed below:

  • New Year’s Day (January 1)
  • Martin Luther King Day (third Monday in January)
  • Memorial Day (last Monday in May)
  • Independence Day (July 4)
  • Labor Day (first Monday in September)
  • Veterans Day (November 11)
  • Thanksgiving Day (fourth Thursday in November)
  • Day after Thanksgiving (fourth Friday in November)
  • Christmas Eve (December 24)
  • Christmas Day (December 25)

WORKER’S COMPENSATION

Employees who are injured or become ill due to work-related reasons are insured by the state worker’s compensation plan.

OTHER BENEFITS

Employee Assistance Program

INTO OSU offers an Employee Assistant Program (EAP) to provide short-term effective, professional, and confidential counselling services for regular full-time employees. A limited number of referral/counselling visits per calendar year per family are provided free of charge for personal or job-related issues. Visit the Human Resources office for additional information.

Educational Assistance Policy

INTO will reimburse expenses incurred up to a maximum of $1000 per fiscal year for continuing education through an accredited program that offers either growth in an area related to his/her current position, or opportunity that might lead to promotional opportunities.  The employee must secure a passing grade of “B” or better for graduate courses, or a grade of “C” or better for undergraduate courses) to receive any reimbursement.  Expenses must be validated by receipts and a copy of the final grade card.  Please contact HR for a copy of the policy and how to apply.
