Stacy Ramirez Nominated for PSPC Exceptional Team Award

Congratulations to Dr. Stacy Ramirez, and her team at Community Health Centers for Benton and Linn Counties, for her nomination for the Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) Exceptional Team Award.

To qualify for a nomination, each team must first participate in progress tracking polls and display exemplary team growth in the PSPC color tier.  Nominations are required to show how a team: collaborates together to support the work of PSPC; engages with partners and stakeholders to advance the team’s care delivery system; demonstrates progress in sustaining an Integrated Medication Management (IMM) program; and has documented improvements in health outcomes and safety.

Now in its fifth year, the Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) is a community of health care providers working in teams to transform the quality of healthcare across America. PSPC teams are expected to work towards improving patient safety and incorporating medication management services in communities across the country. These teams are at the forefront of changing the primary healthcare delivery system into patient-centered health homes.
