ESOL Endorsement Program Application. Submit this application if you plan to complete all ESOL endorsement requirements.
Internship Placement Requests. This form should be submitted several weeks before the requested term for completing ED 410/510 ESOL Internship. Chose the correct form for your teaching status.
- ESOL-Internship-Placement-Request-Licensed (for candidates who are adding the ESOL endorsement to their current Oregon teaching license)
- ESOL-Internship-Placement-Request-Pre-Service (for candidates who are co-enrolled in an OSU preliminary licensure program in Corvallis or the MAT Clinical Practices program)
- ESOL Internship Placement Request – Cascades Preservice Teacher Candidates (for candidates who are co-enrolled in an OSU preliminary licensure program on Cascades campus)
Academic Petition Form. This form is required as part of the process for requesting to substitute coursework from another university or otherwise deviate from the standard program requirements.
