Soil & Testing

Request for Soil pH Test
Print out and fill in the following form. Bring it to the OSU Extension Office in Gold Beach with your soil sample and fee.


School District Background Forms

Port Orford – Langlois School District (Driftwood Elementary & Pacific Highschool)

  • Print and complete the form found on their website HERE
  • Fill out the form and send to Stephanie Smith by email at stephanie.smith@2cj.k12.or.us or drop it off in person at the district office: 45525 Highway 101, Sixes, OR 97476.

Central Curry School District (Riley Creek Elementary & Gold Beach High School)

  • Complete both forms found on their website HERE
  • The Volunteer Application form must be printed and filled out by hand.
  • The Background Check form must be filled out online, printed, and signed by hand (handwritten submissions will not be accepted). 
  • Drop off the forms in person at Riley Creek, GBHS, or the CCSD district office.

Brookings-Harbor School District (Kalmiopsis Elementary)

  • Complete the online form found on their website HERE
  • This form is completely online!
