Fred Meyer will offer the FluBlok Trivalent vaccine and the 2024-2025 COVID vaccine (Pfizer\u2019s Comirnaty or Moderna\u2019s Spikevax, depending on product availability).&nbsp;Vaccines are first-come, first-serve, with limited COVID availability.&nbsp;<\/p>\n\n\n\n<p><strong>Clinic Dates and Location<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>October 3, 2024, Memorial Union Ballroom, OSU Corvallis campus<\/li>\n\n\n\n<li>October 17, 2024, Memorial Union Ballroom, OSU Corvallis campus<\/li>\n<\/ul>\n\n\n\n<p>Fred Meyer pharmacy staff will assist. The process should take under 10 minutes if participants complete the online form process, including the insurance section and active pharmacy billing information. Fred Meyer recommends waiting on-site for 15 to 30 minutes following vaccination.<\/p>\n\n\n\n<p><strong>Insurance Information<\/strong><\/p>\n\n\n\n<p>Participants must bring their insurance cards to their appointments. Fred Meyer accepts most insurance carriers, except Express Scripts, ESI, or Cigna, or subsidiaries or partners of Cigna\/Express Scripts.&nbsp;<\/p>\n\n\n\n<p><strong>Registration<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Register for the October 3 clinic by September 26 at 5 p.m.\u00a0<\/li>\n\n\n\n<li>Registration for the October 17 clinic will open after the October 3 clinic. Employee Benefits will send a reminder.\u00a0<\/li>\n<\/ul>\n\n\n\n<p><strong>Registration Instructions&nbsp;<\/strong><\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li>Enter the benefit code (in ALL CAPS): OSUVAC24<\/li>\n\n\n\n<li>Enter \u2018Vaccines,\u2019 \u2018Corvallis, Oregon,\u2019 and \u201810\/3\/2024\u2019 then click Find Appointment<\/li>\n\n\n\n<li>Confirm the location is 2501 Southwest Jefferson Way. Kaiser has also partnered with Albertsons\/Safeway and Fred Meyer for employer-sponsored flu shot clinics.
