Located in Roseburg OR, Aviva Health is a non-profit Federally Qualified Health Center that delivers medical, dental, behavioral health, and speciality care to more than 15,000 community members annually from nine clinical sites across the area.
But we’re more than a health center – we’re change agents addressing needs that go beyond patient care. With an eye on healthcare workforce development, Aviva Health launched the country’s only family medicine residency program, an effort focused on tackling provider shortages in rural areas of the state.
Equally important is the effort to grow health administration leaders who understand the unique challenges of delivering quality care in rural areas and capitalizing on opportunities as they come available. Types of projects:
Workforce development
- Collaborate with a cross-functional team of leaders, data gurus and strategists to level up our workforce development efforts
 - Assess community needs for advanced practice providers in specialty care practices and create a fellowship program to address those needs
 - Design a geriatric fellowship track for graduating family practice resident physicians
 
Local Speciality Care Needs
- Conduct an assessment to identify gaps in the local market related to wraparound services for children and aging adults with complex needs
 - Work with specialty care and clinical operations team to build efficiences into our women’s health care-delivery model
 
Social Determinants of Health
- Identify and analyze disparities in health outcomes among different racila, ethic, and socio-economic populations within Douglas County and work in cooperation with our public health and SDoH teams to develop a strategy to address those needs
 - Develop strategies to meaningfully incorporate social services in the family medicine footprint to address over-represented, non-healthcare related issues our patient population faces
 
Operations
- Identity two operational areas within the organization that would benefit from the incorporation of AI into their workflows and design a system to integrate and implement the solutation.
 - Collaborate with the clinical operations team to evaluate KPI across multiple departments, establish a process for assessing results and develop recommendations to address negative deviations from optimal outcomes.
