  • A ‘Licensure Request Form’ (top half only) and a check for $100 made out to “Oregon State University”
  • Your ‘Work Sample Documentation Form’.

Here’s the plan for turning in your spring work samples.

Dates:

  • Completed work sample due Monday, June 4th.  (e-mail me if you have a very late work sample and need an additional day or two).
  • The faculty will evaluate your work samples by the end of that week and get them back to you for changes if necessary.
  • Any revisions will be due the following Monday to ensure your licensure paperwork go in on time.
  • If you get done early, please just e-mail when your Docs folder is ready to go so we can evaluate early.

Be sure to check the handbook work sample section for more information.

Once you have passed your spring work sample, you will need to burn a CD of both your fall and spring work samples in separate folders.

Here is the Work Sample Signature Form for spring term.  Please be in touch with your supervisor to work out a timeline for getting this to me (before you start your work sample!).

And the College of Education is planning a special graduation ceremony on the Friday evening (4pm) before commencement.  I don’t have any real information in it yet, but plan accordingly!
